My Chart SSMHC

• “My Chart” from SSM Health lets healthcare patients communicate with their doctor, pay their bill, request an appointment to see a doctor, access test results, request a prescription refill and more

Your Health File

• Access an existing health file account online or register for a new account (for first time users)
• Returning Your Health File customers can sign in with a username and password while new customers will have to go through a short registration process
